Responsibilities:
Provide direct patient care according to the established Plan of Care Suggest changes in Plan of Care to Supervising RN in response to identified patient care concerns Provides accurate and timely documentation consistent with the Plan of Care.
Communicate effectively with all team members, patients and families in order to achieve best patient outcomes.
Requirements:
Valid license as a Licensed Practical Nurse (LPN) Graduate of an accredited school of nursing Minimum 1 year of clinical experience, home care or acute care preferred Valid Driver's License, auto insurance and reliable transportation Excellent observation, problem solving and communication skills Compassion and integrity Job Type:
